Welcome, Volunteers!
At Lebanon Public Schools we love our Volunteers and Watch DOGS. There are a lot of ways you can help us and we feel every person has a talent that can be utilized somewhere. Below are forms that we require to be completed prior to becoming a volunteer or Watch DOG. Please complete and return to your child's school or any school in the district.
Background Check - Student Safety is very important to us. Anyone volunteering in the district is required to complete a background check. Please click here for instructions, and we appreciate you taking time for our students!
VIPS/Watch D.O.G.S. Registration Form - complete and return to school.
Volunteer Covenant - complete and return to school.
Workers Compensation - complete and return to school.
The last three forms must be returned together for quicker processing. If you have children in more than one building, please only fill out one set of forms. Each building will be notified of your clearance to work.
