NATIONAL SCHOOL LUNCH AND BREAKFAST PROGRAM
We encourage all parents to consider filling out this form. Approval is based not only on income, but also on household size. These forms can be returned to any School office, cafeteria or to our central office. To eliminate any possible delays in being approved please fill out only one application per household. Complete all sections of the application and make sure the application is legible and free of errors.
